There are two types of modifiers in this game: exclusive and attack. Exclusive modifiers consist of mana burn effects, chain lightning procs, and bashes. Exclusive modifiers stack with each other but they do not stack with themselves. For example, you could have a Charged Hammer and a Thunderclaw, but only the Charged Hammer would have a chance of casting its lightning. You can, however, stack a Charged Hammer with a Nullfire Blade and your attacks would both burn mana and have a chance to cast a chain lightning at the same time.
Attack modifiers are now more common, since lifesteal effects were included in the category as of patch 0.3.4. These items don't stack with each other, but, if you have more than one, you can choose which one you want as the active modifier. You can have both attack modifiers and general modifiers; for example, Brutalizer and Frostwolf's Skull would stack together. It is a viable tactic to switch your attack modifier on each attack to get multiple effects on a single target and thus bypass the restriction.

Attack Modifiers
Lifesteal
(Exception to this rule is Abyssal Skull's 15% lifesteal aura that does stack with other attack modifiers since it is instead an aura.)
